AM Home Health provides skilled medical care in the patient’s home for individuals who are homebound and require intermittent clinical services. Care is designed to help patients recover, maintain function, and manage health conditions safely at home.
Our Mission
To provide compassionate and comprehensive healthcare to families in our community — offering home health services, hospice care, and DME with excellence, speed, and human warmth.
Key Services Provided:
1 Skilled Nursing Care
- Medication management and teaching
- Disease and symptom monitoring (e.g., diabetes, heart failure, wound care)
- Injections and medication administration
- Catheter care, ostomy care
- Education for patient and caregivers
2. Therapy Services
- Physical Therapy (PT): Strengthening, mobility training, safety with transfers, fall prevention
- Occupational Therapy (OT): Assistance with activities of daily living (ADLs), energy conservation, home safety adaptations
- Speech Language Pathology (SLP): Swallowing therapy, speech/cognition rehabilitation, communication strategies
3. Home Health Aide Services
- Limited assistance with personal care (bathing, grooming, dressing) under the supervision of nursing/therapy
- Note: This is not long-term or continuous custodial care
4. Medical Social Work
- Support with coping and adjustment to illness
- Resource coordination (e.g., community programs, financial/insurance guidance, advance care planning)
5. Care Coordination with the Patient’s Physician
- Regular updates to the physician
- Adjustments to care plan based on clinical changes
What Home Health Does Not Provide
- 24-hour care
- Continuous custodial/personal care
- Homemaking services (cleaning, shopping, cooking) unless tied to skills training
